BMC 15^th Anniversary T-Shirt Purchase Order

 

If we can get an order for 100 shirts, the cost will be $10.00 per shirt.  The shirts are Gildan Adult Ultra Cotton Tees, 100% pre-shrunk cotton.  All shirts will be white with your choice of car color, style of car, and size of course.  The car color will also be the color of the word Miata on the 15^th Ann. Logo on the front of the shirt.  Please fill out the form below and return it with payment to Rob & Amy Young on or before the April meeting, 4/7/05.  Send to: 6022 Linworth Rd , Worthington , OH 43085 .  You can see a picture of the artwork on the classified page of the clubs web site.  Checks should be payable to "Buckeye Miata Club".
